We reduce the risks associated with alcohol and promote change to improve the health and social welfare of patients. The Community Alcohol Support Worker (CHAT) will support the alcohol detox nurses within the team. This post is part-time, 30 hours per week.
We offer a 7-day nurse-led service, and our hours are 8 am - 4 pm, providing increased support as required.
We provide psychosocial interventions for up to 12 weeks from discharge at UHNM, focusing on harm reduction, relapse prevention, and seamless referrals to recovery services.
Main Duties of the Job
Support earlier discharge from the hospital/admission avoidance for suitable patients with alcohol-related problems.
Assist service users who are utilizing multiple emergency services (e.g., ambulance, A&E, IOU, Harplands, crisis teams).
Provide increased support for service users with co-existing complex physical health needs (e.g., chronic liver disease, end of life) that require a coordinated case management approach.
Deliver increased support for people known to be difficult to engage in services and who require an assertive management approach (e.g., home visits, greater number of contacts to remain engaged).
Assist registered professionals in assessments and care planning/implementation and evaluation of individualized care plans.

Job Responsibilities
Communicate with service users, carers, all members of the multi-disciplinary team, and staff in external agencies using various methods including Information Technology.
Regularly report and effectively communicate service users' health status and care aims in the Halo System, providing verbal reports to registered health professional staff members and other members of the multi-disciplinary team.
Maintain professional boundaries while relating appropriately with service users, carers, colleagues, and others at all times.

Identify, minimize, and report risks of danger, harm, and abuse upwards to senior colleagues.

Undertake elements of a specific alcohol and holistic assessment under the direction of registered health professional staff.
Work in partnership with service users to assist them in identifying recovery planning that is meaningful to them and supports agreed health outcomes.
Support service users in accessing a range of activities and services to maintain their independence and promote their health and wellbeing.
Provide advice and guidance to service users, promoting healthy activities and lifestyle choices.
Support registered practitioners and service users during activities to meet their physical substance misuse and mental wellbeing needs.
Support service users in meeting their assessed personal care needs.
Provide information, support, and guidance to carers, friends, and relatives of service users, maintaining appropriate confidentiality.
Take responsibility for personal and professional development and support the development of colleagues, students, trainees, and volunteers.
Access and make use of supervision regularly.
Participate in activities focused on delivering a high-quality customer-focused service.
Undertake activities to improve services and make suggestions for enhancements.
Person Specification Qualifications
Advanced Apprenticeship in Mental Health and Social Care or NVQ 3 in substance misuse with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or Functional Skills).
Experience
Experience of working with people with substance misuse needs as a member of a multi-disciplinary team.
