As a member of our busy Carer Support Team within Adult Social Care, you will contribute to creating a carer-friendly community in Shropshire.
The main focus of this role will be undertaking face-to-face carer assessments with carers from community hubs around the county. You will contribute to establishing, reviewing, and developing this project, initially for 12 months, to pilot face-to-face carer assessments (Care Act 2014) being undertaken by the Carer Support Team.
Working with the carer, you will identify their ongoing support needs and significantly contribute to delivering support and information to family and unpaid carers, enabling them to achieve their own outcomes.
