We are looking for an enthusiastic, committed practice nurse to join our large, multi-disciplinary primary care team to provide high quality, compassionate care to our patient population. We are a large practice delivering care to a total of around 26,000 patients across four sites with a nursing team of Practice Nurses, Nursing Associates, Healthcare Assistants, and a Phlebotomist.
We are looking for a practice nurse, ideally already with an area of interest in Chronic Disease Management. As a practice, we have a strong record in training and development, though we would consider a candidate from outside of primary care with the right attitude towards patient care and developing their career within General Practice.
You will need a caring and compassionate nature, excellent organisational skills, and to enjoy working with people. The world of General Practice is busy and ever-changing, so an ability to learn quickly and be adaptable to rapid change is essential. The role will require an ability to organise and prioritise your own workload, and so you should be comfortable working independently and as a committed member of a multi-disciplinary team.
Main duties of the job
To provide high quality, compassionate nursing care to our patient population: To work within the multidisciplinary team of GPs, nurses and allied health professionals to deliver preventative medicine and chronic disease management to our patients. To provide nursing input into practice policy development and practice decisions affecting the care of patients. To contribute to QOF, practice research, audits and other local or national directives. Provide high quality, compassionate nursing care to our patient populations alongside the nursing and healthcare assistant team. Undertake Practice Nursing duties including, but not limited to: Cervical Screening Childhood immunisations Ear Syringing ECGs Audiology Venepuncture Take a lead in an area of Chronic Disease Management, such as Asthma, COPD or Diabetes. Work alongside GP leads to deliver practice targets such as immunisation uptake. To assess, diagnose and prescribe appropriately within your capability and nursing grade, ensuring practice prescribing policy is adhered to. About us

Area of chronic disease management expertise preferably diabetes. Experience of working within General Practice. Disclosure and Barring Service Check
