Park View Surgery and The Nayar Practice are looking for a Care Coordinator who can provide some administrative duties to join our friendly team. The successful candidate will be the first point of contact where they will greet patients, staff and service users. The post holder will be involved in the day-to-day running of the reception ensuring that patients are directed to their appointments in a timely manner. Main duties of the job

Main duties of the job include, but are not limited to: Ensuring an effective and efficient reception service is provided to patients and other visitors. Providing care navigation/signposting to patients. Dealing with all general enquiries, face to face, telephone, emails & booking of appointments. Liaising with other health care staff outside of the practice. Receiving and making telephone calls as required, taking messages accurately. Entering data into the clinical system, ensuring to have carefully recorded relevant details. Ensuring efficient running of the appointment system and monitoring flow of patients. Being flexible, confident, and enthusiastic whilst being able to demonstrate a high level of accuracy. Projecting a positive and friendly image. About us

Both The Nayar Practice and Park View Surgery are well-established Practices based in Edlington & Sprotbrough with Park View Surgery rated outstanding and The Nayar Practice Good by CQC. Both Practices form part of the North Primary Care Network, together with a population list size of over 8,000. Our patients give us excellent feedback and our list size is continually increasing. The Practice teams comprise of 1 GP Principal, regular locum GPs, ANPs, Practice nurses, 1 Dermatology Specialist Nurse, Health Care Assistants, 1 Nurse associate, Practice Care Coordinators and Administrators. Job responsibilities

You will be responsible for the day-to-day running of reception, ensuring that an effective and efficient service is being provided. You will care navigate/signpost patients to the most appropriate service for their needs, dealing with all general enquiries, booking of appointments and using your own judgment and communication skills to ensure that patients see the right clinician at the right time. This role is ideal for someone who is highly organised, an effective communicator, who can effectively problem solve, and work as part of a team whilst providing a professional and warm welcome to our patients. The post holder will have excellent communication skills, a compassionate and friendly nature and be able to operate in a busy ever-changing environment with a range of people and professions. The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice health & safety policy, the practice health & safety manual, and the practice infection control policy and published procedures. This will include: Using personal security systems within the workplace according to practice guidelines. Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks. Making effective use of training to update knowledge and skills. Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards. Actively reporting health and safety hazards and infection hazards immediately when recognised. Keeping own work areas and general/patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holder's role. Undertaking periodic infection control training (minimum annually). Demonstrating due regard for safeguarding and promoting the welfare of children. Equality and diversity

Qualifications

GCSE grade A to C in English and Maths. Experience

Experience of working with the general public. Experience of working in General Practice. Knowledge of Emis. Disclosure and Barring Service Check
