Reporting to the Primary Health Care Manager (PHC), the Primary Health Care Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management, and Department of Family Practice (DFP) portfolios specifically related to the health and wellness of Men of African Descent. The PHC Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives assigned through leadership in the Nova Scotia Brotherhood portfolio. The scope of practice for the PHC Coordinator will include collaborative work with African Nova Scotian leaders and community. Liaise with health care providers to support change management strategies that are inherent in the services plan for primary health care. The PHC Coordinator is responsible for supporting the overall coordination of the planning, development and implementation of the activities assigned under the direction of the leadership team. Please note travel within Colchester and Pictou County area may be required. A valid Nova Scotia Driver’s License is required. About You

We would love to hear from you if you have the following: Bachelors degree preferably from a health discipline/health related science Current registration with professional association if applicable Two years' of recent related experience An equivalent combination of education and experience may be considered Experience working in a family practice setting with family physicians an asset Experience working with African Nova Scotian community considered an asset Coordination and Project Management Skills Broad knowledge of the health system with high level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management Experience in Practice Facilitation including coaching, facilitation, change management strategies, and quality improvement is an asset Organizational and process management skills Superior time management skills in order to prioritize and complete assignments Ability to think critically, quickly and accurately; creative problem-solving ability Demonstrated leadership in facilitation and multiple stakeholders and service providers Proven ability to build, enhance, foster and maintain effective relationships Proficiency in Microsoft Office Proficiency in cultural competencies Must have a high level of judgment in order to make independent decisions related to responsibilities Competencies in other languages an asset, French preferred Hours

Permanent full-time position; 70 hours bi-weekly. Shifts may include evenings, weekends and holidays Compensation and Benefits
