We have a fantastic opportunity for a highly motivated dynamic Section 117 aftercare case manager to join our clinical/Nursing team and bring forward their expert leadership and management experience. Applications will be welcomed from individuals who already have experience in management and leadership roles in Section 117 aftercare. The post holder is responsible for overseeing the clinical governance of the cohort of individuals that receive packages of care funded under Section 117 aftercare. The primary duty of the post holder will be to manage the clinical governance of the Section 117 case load, including managing all referrals for Section 117 aftercare that are submitted to the ICB and ensuring the care and support plans from providers are in line with the assessed Section 117 aftercare needs. Main Duties of the Job

The successful candidate will be responsible for providing management, clinical governance, and overseeing Wakefield's Section 117 aftercare caseload. This includes working in partnership with the local authority to ensure Wakefield's statutory responsibilities under Section 117 aftercare are met. The remit includes the development, implementation, evaluation, and review of Section 117 programmes of care, as well as the administration of processes to designate and allocate and to advise on/managing patients. The post holder will work as part of the clinical team to develop the role and contribute to the development and implementation of robust Section 117 aftercare processes. They will also support the development of integrated commissioning arrangements with the Wakefield Local Authority, including Community Based Commissioning arrangements and associated strategies. The post holder is responsible for overseeing the clinical governance of the cohort of individuals that receive packages of care funded under Section 117 aftercare. This includes managing all referrals for Section 117 aftercare submitted to the ICB and ensuring care and support plans from providers align with assessed Section 117 aftercare needs. The post holder will work jointly with the local authority using agreed funding tools to establish appropriate funding splits and will support the Care Programme Approach planning and delivery. They will also complete Section 117 aftercare funding applications and provide recommendations to establish patient eligibility for NHS funding or act as a decision maker. A key role is supporting Local Authority and NHS commissioned services representatives to deliver on timely and appropriate care. Person Specification

Qualifications

Educated to post-registration/graduate level or equivalent in experience Hold a RNLD or RMN Registered Nurse Qualification Current and relevant NMC registration Recognised teaching/mentoring qualification or equivalent experience Skills & Behaviours

Able to analyse and interpret complex business data Able to deal with difficult and contentious issues in a professional manner, ensuring the correct courses of action are undertaken Skills for managing projects, ensuring they remain on track and within budget Able to effectively manage allocated resources within the team and identify potential issues around demand and capacity, ensuring timely solutions Experience

Significant experience of both community and hospital working and a sound understanding of Section 117 aftercare Clinical experience of working with children with mental health and/or learning disabilities Evidence of cross-sector partnership working Working knowledge of functions within a large and complex organisation Additional Requirements

Excellent communication skills including writing, report writing, and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences Disclosure and Barring Service Check
