Our Home First & Reablement service supports people return home after a stay in hospital. This helps them continue their recovery in their own surroundings - regaining their ability to live as independently as possible. We are recruiting experienced and new to care Rehabilitation Assistants in Gloucester. As a Health Care Support Worker, you will work closely with other health care professionals, delivering essential clinical and personal care. You will ensure that our service user needs are met and assist the team in maintaining high standards of care delivery. Previous experience is desirable but not essential. If you are new to care and hold no experience in healthcare, then we offer a fantastic development opportunity with a structured development plan (including achieving the Care Certificate). You will progress from a pay band 2 to pay band 3 on achievement of the Care Certification. Our service runs 24/7 and all our Rehabilitation Assistant roles offer core hours of 07.00-22.00. Full-time and part time roles operate on a 4-days on, 4-days off schedule. This post has a part time role available, 21 hours per week, covering morning shifts. Main duties of the job
You don't need any previous caring experience - all we ask is that you: enjoy working with people; understand what motivates people; are comfortable being involved in practical, sometimes personal and intimate care provision for people; have a desire to learn and develop skills; are able to work alone and in a team setting. The support we provide includes washing, dressing, cooking meals, accessing community services, arranging delivery services, improving mobility and balance so patients can safely use steps or stairs, walk in and outdoors, or use transport. As this is a community-based role a full Driving Licence valid for the UK and access to a vehicle insured for business use is essential. Job responsibilities
Promote people to live safely and confidently at home, working with the team, including: Providing practical, personal and intimate care whilst maintaining high levels of hygiene and health and safety; Assisting the person to work on outcome focussed goals enabling them to relearn or develop new skills and abilities required for daily living; Monitoring progress and function of people within their reablement programme and contributing to the ongoing assessment of their rehabilitation needs by liaising with team colleagues and recording progress, in compliance with organisation policies and procedures; Identifying when a safeguarding alert may need to be raised, ensuring the timely escalation of suspected abuse and self-neglect promptly; Supporting people and where appropriate their families and carers, by advising them on all aspects of their Reablement plan including the correct use of equipment; Adhering to all health and safety instructions. Report to line manager any incidents, emergencies or accidents; Connecting people to community resources, promoting digital inclusion and the use of assisted technology e.g. telecare; Being an active participant of your team, maintaining your own continuing professional development, to ensure you can deliver the best quality care and support to the people of Gloucestershire.
