The main duties will include supporting the well established and very experienced practice nursing team to deliver reviews and care plans for patients with long term conditions such as Asthma, COPD and Diabetes. About us
We are a multidisciplinary primary care team who value nursing as a key component to the successful running of the practice. Input and ideas particularly from our nursing team members about how we can continue to improve the care we offer is essential. The practice as a whole is among the highest performing practices locally, driven by patient satisfaction and patient access to the highest possible standards of care. The team includes 2 Practice Nurses, a Nurse Associate, 2 Health Care Assistants and 4 Advanced Nurse Practitioners. We have 4 GP Partners, 2 Salaried GPs all of whom work alongside our extended Primary Care team of Pharmacists, Pharmacy Technicians, First Contact Practitioners, Mental Health Nurse, GP Assistants, Care Co-ordinators and Health and Wellbeing Coaches. As a Training Practice we also support GP Registrar Placements, placements for medical students as well as student nurses and other apprenticeships. Job responsibilities

Qualifications
Registered Nurse - Adult, Nursing and Midwifery Council Register At least 1 of the following: Diabetes, Asthma and COPD certificate or diploma Diabetes, Asthma and COPD certificate Disclosure and Barring Service Check
