We're seeking a dedicated Senior Physical Therapist to join our team in Wakefield, Rhode Island. In this role, you'll provide comprehensive physical therapy services to residents in our Skilled Nursing Facility, ensuring their health, well-being, and functional improvement. Key Responsibilities

Develop and implement individualized treatment plans based on physician orders Conduct thorough evaluations and maintain detailed progress notes Collaborate with interdisciplinary teams to enhance resident care Participate in care planning meetings and provide input on resident needs Maintain accurate statistical records of treatments Contribute to in-service training programs Supervise Physical Therapy Aides during Director absences Ensure compliance with all regulatory standards and best practices Qualifications and Requirements

Graduate of an accredited Physical Therapy program Current Rhode Island Physical Therapy license 2+ years of experience in physical therapy (preferred) Passion for geriatric and subacute rehabilitation Strong leadership and communication skills Proficiency in electronic health records systems Commitment to continuous professional development About Wakefield, Rhode Island
