Nurse Practitioner – Hospital at Home

The Hospital at Home service provides secondary level care within the patient’s own home or care setting. The Nurse Practitioner will work as part of the Hospital at Home team providing clinical care to patients who would otherwise be in a hospital setting. This post is integral to ensuring safe, effective, high-quality care delivery in the patient’s home environment. The nurse practitioner will liaise with several professionals/agencies, both internal and external, daily including GP practices, local authority teams, and the voluntary sector. The nurse practitioner will help towards continually developing a sustainable and appropriately skilled workforce with a reduction in dependency on conventional medical models of care provision. Currently, the service is 7 days, 0800-1800 daily. The post holder for the post would work Monday – Friday with weekend working. This reduction will also be applied pro rata for part-time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37-hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period, for which you would be paid, until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
