We are currently recruiting for a dedicated and motivated Band 6 Senior Staff Nurse to join our community nursing team covering the borough of Spelthorne.

The Community Team visits patients in their own home as well as care homes. There are excellent opportunities for training and development of both your clinical and leadership skills. The team has experienced Senior District Nurses who will ensure that you have the necessary support for your personal development. The community nursing service works over a 7-day week from 8.00am to 8.00pm, offering a variable shift pattern, and includes weekend and bank holiday working.

Individuals will be expected to deliver and maintain a high standard of clinical and professional Nursing Care and to assist in the delivery of all aspects of individualised patient care.

Job responsibilities Responsible for the delivery of care and for managing service provision in accordance with national and local priorities, organisation policies, procedures, and guidelines and in accordance with the Nursing and Midwifery Council.

Principal duties include:

Responsibility for a defined caseload of patients

Named District Nurse for designated GP practices and attending all GSF and MDT meetings

Undertaking full nursing assessments of patients, including those with multiple pathology and complex health and social needs

Agreeing the patients' care plan, liaising with others, and adapting the care and treatment delivered according to need and patient choice

Monitoring medication regimes, assessing effects, and advising patients on the safe storage and disposal of drugs

Ensuring the correct use of aids and equipment supplied for patients and carers and maintaining standards of infection control and safety

Recognising own limitations in clinical care and urgency of patients' needs, referring to other health care professionals accordingly

Maintaining accurate and legible patient records in line with professional and organisational requirements

Making autonomous clinical decisions in planning and prioritising patient care

Recognising the needs of the service, identifying risk, and making rapid autonomous decisions

Triaging within the District Nurse Referral Centre and promoting technology within the workplace

Operational line manager for District Nursing team (Band 5 RNs / Band 4 APs / Band 3 HCAs)

Effectively communicating with others involved in patient care at all times

Maintaining strong working relationships with GPs, practice nurses, social services, and other stakeholders

Completing all own mandatory training and ensuring it is completed for all team members

Participating in own annual appraisal and undertaking continuing professional development

Supervising and appraising team members

Teaching, supervising, and mentoring other team members and pre-registration students

Participating in clinical audit and implementing changes to clinical practice

Actively participating in meetings and working groups

Involved in the development and implementation of community nursing service developments

Acting as a link nurse for defined areas (e.g., tissue viability, end of life, diabetes)

Responsible for ordering and stock management of dressings and other equipment

Person Specification Qualifications

Current 1st level registration (Registered Nurse)

Degree in relevant health care (or working towards)

Evidence of Continued Professional Development post registration

Relevant specialist courses Clinical Assessor and Supervisor Clinical Supervisor

Educated to Degree / Master level Physical Assessment Module Nurse prescriber (DN V100)

£39,205 to £47,084 a year inclusive of 5% High Cost Area allowance per annum pro rata.
