Two Rivers Medical Centre in Ipswich is looking to recruit an advanced care practitioner to join our multi-disciplinary team to help us transform and modernise pathways of care. This role is offered on a part or full-time basis, with flexibility to days and hours. As an experienced healthcare professional, educated to masters level, the successful candidate will have developed the skills and knowledge to take on an expanded role and scope of practice caring for patients. They will demonstrate flexibility in attitude and behaviour and will be keen to undertake new ways of working and delivering healthcare, committing to the professional development required to adapt to changing circumstances. Main duties of the job

Demonstrate a high level of expertise within the specialist service providing advice, education, and support to staff, patients, their families, and carers. Practise autonomously as an advanced care practitioner within the designated specialty area to provide patient-centred clinical care, encompassing advanced clinical assessment, examination, diagnosis, and treatment within an agreed scope of practice throughout the directorate. Support new ways of working that emphasise a more efficient and patient-focused service, ensuring the safe treatment, referral, and discharge of patients with undifferentiated and undiagnosed presentations in their area. Lead the ongoing development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols, and guidelines in collaboration with multidisciplinary colleagues. Qualifications

Professional registration with relevant professional body: NMC, HCPC, or GPhC. Post-graduate qualification in specific specialty, or equivalent experience. Post-registration experience at a senior level (Band 7 or above) in specialty area. Experience

Evidence of ongoing CPD. Advanced clinical assessment skills. Ability to perform in stressful and complex situations. Sound clinical reasoning and decision-making. Ability to inspire and lead a team. Disclosure and Barring Service Check
