Mental Health Liaison Practitioner

with the service based at

Stepping Hill Hospital in Stockport.

If you are a registered mental health professional with experience in assessing and supporting individuals of any age and you would like to be part of a growing and developing service, we would like to hear from you. We are particularly interested in colleagues with experience working with both working-age and older adults to support our urgent pathway within the team and individuals with a background in crisis and acute care to support our emergency pathway.

Main duties of the job

As part of the Mental Health Liaison service, you can expect professional support and development in this exciting and challenging area of work. You will be joining a friendly and supportive team at Stepping Hill Hospital and will receive regular management and clinical supervision and have opportunities to engage in CPD. As a valued member of the liaison team, you will be fully involved in all service developments and given opportunities to support quality improvement. Significant developments have taken place over the past 12 months with acute trust colleagues supported by NHS England, which means this is a great time to join the team. We are also currently undergoing the PLAN accreditation process with the Royal College of Psychiatrists.

The service runs 7 days a week to support wards and 24/7 to support the Emergency Department, so posts will be worked on a shift basis to include nights, weekends, and bank holidays.

Person Specification

Education/Qualifications

Essential

Professional registration in health or social care with up-to-date accreditation / CPD - RMN, Dip SW, BSc Occupational Therapy or equivalent. Short course(s) and experience to postgraduate diploma level.

Desirable

Educated to degree level Accredited skills-based training Training related to management skills, teaching, staff development, and supervision Non-medical prescriber AMHP

Experience

Essential

Proven experience across a range of acute mental health services Demonstrate working experience of a range of assessment tools for various mental health conditions Previous experience of working effectively with other disciplines

Desirable

Experience of working within liaison services Experience of working with older adults with functional illness and/or organic conditions in either an inpatient or community setting Experience in identifying and managing delirium Experience of supporting individuals who misuse alcohol or drugs Experience of delivering training/teaching to other professionals Experience of service development/managing change

Knowledge

Essential

Sound knowledge of evidence-based contemporary mental health practice. Sound knowledge base of holistic risk assessment and management. Good understanding of the impact of physical and mental health co-morbidities Sound working knowledge of Mental Health Act and Mental Capacity Act legislation and DOLS

Desirable

Knowledge of the range of services available for individuals locally Awareness of a range of evidence-based interventions Awareness of the value of research/audit and quality improvement

Skills

Essential

Skills in Assessment/Care Management and planning Ability and skills to undertake comprehensive risk assessment and formulation of risk management plans including supporting patients who self-harm or are in a neglectful state Ability to undertake Bio/Psychosocial assessments across a range of patient groups and age profiles Excellent verbal and written communication skills with a wide range of people including patients, carers, and other professionals Ability to work autonomously, prioritize, and manage own workload Good organizational, problem-solving, and time management skills and an ability to work under pressure Ability to supervise staff, set standards, monitor performance, and promote staff development Possess a positive attitude to change and commitment to service development Be able to work as an effective member of the multidisciplinary team Be able to work a range of shifts over a 24-hour, 7-day period on a rotational basis Ability to formulate clear discharge plans across a range of patient groups and age profiles
