The role: As a District Nurse, you will be working in partnership with patients and their families to deliver person-centred, safe, and effective care in the community. You will work as part of the District Nursing team and have responsibility for a delegated caseload. The role will require you to undertake evaluation of patient care needs and ensure care plans are updated and accurate at all times, ensuring patients are able to remain independent in their own homes. You will liaise with all healthcare professionals and other agencies to ensure effective service delivery. As a community nurse, you will thrive in a fast-paced environment.
