Duties:
Assessing and promoting the needs of our elderly residents to enable our residents to live their best life.
Create and follow person-centered care plans to provide the holistic care needed.
Take responsibility for leading the shift effectively and positively.

Excellent communication skills
Knowledge:
Hold a valid NMC registration
Evidence of personal and professional development since registration

Ability to build and maintain positive, professional working relationships
Computer literate
Experience:
Drive and commitment to provide high quality care and exceed expectations
Experience of Dementia/End of Life care
Experience working with an electronic care planning system, preferably PCS
Experience in the development and implementation of person-centered care planning
