JOB TITLE:
Adult Social Worker - Hayes & Five Elms Locality Team We are looking for a social worker to work within Hayes and Five Elms Locality Team. The Locality team works on ongoing care management interventions, and the coordination team leads on annual reviews of support plans, extra care housing support, and direct payment work. This service also provides assessments under the Care Act 2014 and works with young people transitioning from Children's services to Adult services and beyond. You will be supporting younger adults with physical disabilities and sensory impairments, and older people to live as independently as they are able, while working with their carers to ensure risks are appropriately managed. You will need to be flexible, committed, and hardworking with a willingness to work to quality standards. You will be working within a multidisciplinary framework and need to have good communication and negotiating skills. You will be working with partner agencies and will be required to develop close working relationships with them. The successful candidate will be required to work as a duty worker on a rota basis, facilitate hospital discharges, undertake safeguarding (s42) enquiries, and conduct mental capacity assessments/best interest decisions. We are looking to appoint someone who is suitably qualified, innovative, enthusiastic, motivated, and able to work well as a member of the team. Flexible Working:
