Sirona Medical is recruiting for a Reablement Practice Manager.
Responsibilities:
Direct supervision and approval of Reablement goal plans and Care Act assessments of approx. 5-6 staff (Reablement officers, Duty SW).
Monthly 121’s, appraisals, system approvals, case discussions/problem solving as required.
Provide legislative guidance and lead any process development or mini projects.
Investigate and respond to any complaints/ME’s.
Utilize good digital/IT skills and conduct joint visits if required to support robust practice.
Represent the team where required and attend panel meetings as necessary.
Engage with practice improvement/CQC visit preparation as required.
The Social Worker will need to be experienced as a manager and possess robust and practical knowledge/experience of Reablement.
Work Arrangement:
Mostly remote working with office-based work one day per week. Minimum Requirements: Fully qualified (DipSW, CQSW, CSS or equivalent). Social Work England Registered. Must have physical and older people Reablement experience. Recent management experience.
