We are currently seeking an MDS Coordinator with PDPM experience to join our vibrant team.
Key Responsibilities:
Facilitates weekly skilled Medicare meetings and provides clear communication regarding skilled coverage criteria
Transmits MDS to CMS and follows up to validate any errors or messages revealed on final validation reports
Completes PDPM/MDS assessments within regulatory time frames including CAAs
Participates and reviews invoices as indicated for skilled patients under consolidated billing
Maintains a current PDPM schedule while setting all PDPM MDS ARDs within time frames in the MDS software program
Completes MDS accuracy/PDPM reviews with required software/process in a timely manner
Participates in Triple check/Compliance reviews
Monitors skilled documentation for potential opportunities/changes
Maintains and follows a schedule of due dates for all MDS.
Coordinates the completion of the MDS by all disciplines.
Reviews the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy.
Finalizes and transmits the MDSs per RAI requirements
Completes Medicare Physician Certification and monitors compliance with CMS Medicare guidelines
Verifies that assessments have been transmitted and approved by the State in a timely manner.
Corrects any rejected records and prepares them for re-submission.
Completes CAAs according to state guidelines.
Provides weekly updates for managed care residents.
Minimum Qualifications:
Current RN or LPN license in MA with previous experience in MDS/PDPM (2 years or more)
Previous experience in a hospital, long-term facility, or other healthcare related facility.
Knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care.
Computer skills - Windows applications, computer experience; Accu Med experience preferred.
Previous experience in completing MDS/PDPM is required.
Ability to work independently
Flexibility with schedule for month end close
Detail-oriented and able to accurately and timely complete assessments.
