Provide specialist assessment and care for patients with respiratory conditions, including COPD, asthma, bronchiectasis, and interstitial lung disease, within the Virtual Ward setting. Work with the Virtual Ward hub team who monitor patients remotely using telehealth systems and clinical assessments, ensuring early recognition of deterioration and timely interventions. Utilise point-of-care testing to support clinical decision making. Develop and implement individualised care plans in collaboration with the wider multidisciplinary team, including consultants, GPs, and other community services. Deliver specialist advice, education, and support to patients and their families, empowering them to manage their conditions at home. Work closely with the Virtual Ward team to review and monitor patient progress, adjusting care plans as necessary. Participate in the training and development of colleagues, promoting best practices in respiratory care and virtual ward delivery. Ensure compliance with clinical governance, patient safety protocols, and quality assurance processes.
