This post will have a lead role to develop the physiotherapy practice and deliver evidence-based Inpatient Rehabilitation, enabling people returning home following initial treatment in acute units serving the Barnet locality. Proactively monitor and anticipate the changing needs of the patients, coordinating their care across health and social care systems, and attend MDT meetings. Plan, implement, and review establishing patient-centered goals, personalized specialized programs of health interventions that are preventative, self-managing, and supportive for these patients. This should also encompass crisis plan management for patients and carers where needed.
The role includes in-reaching and triaging acute referrals, coordinating the ward audits, team capacity, and working on service development projects with the wide North Central London Integrated Care system, coordinating student and new recruits.
To be an active member of the multidisciplinary neuro-rehabilitation team, providing highly specialist physiotherapy assessment and treatment. To participate in the delivery and evaluation of the general rehabilitation and Neurorehabilitation pathways. To be responsible for the education and supervision of junior physiotherapy staff, including rotational staff, rehabilitation assistants, and physiotherapy students. Post may require weekend working.
