Provide high-quality occupational therapy for individuals within the community rehabilitation setting, focusing on early discharge and maintaining independence. Conduct assessments and create individualized treatment plans for community rehabilitation and equipment provision. Offer guidance and support to clients in reviewing care packages, ensuring they meet the highest standards. Work collaboratively with a multidisciplinary team to ensure comprehensive and cohesive patient care. Engage in documentation and reporting on patient progress to facilitate successful rehabilitation outcomes. Requirements: Registered with the HCPC. A minimum of two years' experience in community rehabilitation and early discharge. Skills in equipment provision and the ability to review care packages effectively. Car driving is preferable.
