Job summary

We are recruiting for a Specialist Clinical Practitioner (Older Persons Mental Health) to join our team. You should share our passion and drive to ensure that our patients receive quality and compassionate care. You could be a Registered MH Nurse, Occupational Therapist or Social Worker with specialist experience and knowledge in Older Persons Mental Health. Do you have a high level of skill in the assessment and care of people who present to the Emergency Department in mental health crisis and those who have mental health needs during their inpatient stay in the acute hospital? This position is suitable for a highly motivated, compassionate person who can work under pressure. You'd be expected to provide clinical leadership, carry out clinical assessment and intervention, and provide education to the team and acute hospital in the specialty of Older Persons Mental Health. You should have excellent written and verbal communication skills and be able to interact at all levels. This is a challenging but rewarding role, within a busy and evolving multidisciplinary service. Main duties, tasks & skills required

The MH&LD Liaison Team provides assessments, advice, and reasonable adjustments for people with mental health needs and learning disabilities accessing treatment at St Mary's Hospital. We aim to improve the parity of care for people of all ages with mental health and learning disabilities in the acute hospital and work with their family and carers to ensure they have the best possible care and treatment. The MH&LD Liaison Team works across the full age spectrum and provides a 24-hour service across seven days of the week. Additionally, MH&LD Liaison Team collaborates with Ambulance service to support people with mental health needs and learning disabilities in the community through the Mental Health Rapid Response Vehicle service (MHRRV). Qualifications

Essential

Appropriate relevant professional qualification. Masters or Additional postgraduate qualification related to mental health care or equivalent experience. e.g. Mental Health Nurse, Social Worker, Occupational Therapist, with appropriate registration.

Experience

Essential

Evidence of other development and/or training which indicates a commitment to continuous development and preparation for practice at a senior level. Significant post-qualification experience in mental health service. Experience working within older persons mental health care of assessing and managing complex physical and mental health problems, both functional and organic in nature. Experience of working with dementia, recognising through assessment the different aetiologies and appropriate treatment options. Experience of working with delirium, ability to recognise the signs and symptoms, how to rule it out, and treatment options in the acute setting. Experience of managing acute risk in the context of mental illness and psychological distress. Experience of making autonomous decisions regarding discharge, signposting, and admission of service users following a comprehensive assessment and evaluation of risk. Experience of working with service users with harmful use of illicit substances and alcohol. Experience of conducting audits and service evaluations. Experience of service development, Quality Improvement, and leading change. Experience of promoting and maintaining high standards of professionalism and of supervising junior colleagues.
