This post is an exciting opportunity for someone who is keen to take the D2A service in Mid Notts to a new level. We are in the process of improving our D2A service to ensure that we actively support patient flow from hospital to the community, ensuring that patients are discharged in a timely manner, improving health outcomes. You will work closely with Occupational Therapists, Assistant practitioners, support staff, clinical assessors (UCR team), district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes. Main Duties of the Job

An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management, and support timely discharges from our acute Trust partners. You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness, and enable health, wellbeing, and self-care. You will be visiting patients within their own homes using a goal-oriented focus to maximise patient independence, safety, and quality of life. The post will involve cross-site working and community visits. A full UK driving licence and vehicle for business use is essential for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Job Responsibilities

You will manage your own caseload in the community. As part of the team, you will work closely with the rest of the MDT to provide the best possible outcomes for our service users. You will have a degree/diploma in Physiotherapy and be HCPC registered. You should possess excellent interpersonal and time management skills, be a good communicator, and able to prioritise and work well as a member of a team. An induction period and supernumerary support will be given. You will have significant experience of working in the NHS in the UK as a Band 5 Physiotherapist and being part of NHS healthcare services. Person Specification

Qualifications

BSc/Diploma in Physiotherapy HCPC Registration Clinical Educator Relevant courses in Health Care of the Elderly/Falls/Orthopaedics/Palliative Care/MSK Previous Experience

Proven post-qualification experience with some at a more senior level Recent experience in health care of the elderly/falls/orthopaedics/palliative care Supervisory skills Experience of audit Significant experience of working as B5 physio in the UK as part of the NHS Teaching experience Standard setting, service evaluation, recruitment & retention Community experience Pathology of the older person Professional Codes of Conduct Awareness of diversity Evidence-based practice Ability to assess, plan and evaluate rehabilitative treatment Knowledge of management of falls Knowledge of local & National priorities Knowledge of Risk Assessment Treatment of complex cases & diverse conditions Knowledge of equipment/adaptations and all aspects of ADL Organisational skills and use of initiative Self-motivated and able to work as an effective team member Ability to communicate with team, patient/client and their families and other agencies Ability to manage self Flexible and positive approach Demonstrates meeting the requirements of CPD Contractual Requirements
