This is a new and exciting opportunity for an experienced Physiotherapist to lead the inpatient therapy team at Dawlish Hospital and maintain a ward-based clinical caseload.

Dawlish Hospital has a 16-18 bedded ward for patients who have no criteria to reside (NCTR) and require further assessments and treatment to support their discharge. This includes complex discharge planning and the development of both the therapy provision to the ward and an MDT enabling approach to minimize hospital-acquired deconditioning through innovative ways.

On the ward, we are passionate about reducing the harmful effects of deconditioning and ensuring what matters to patients and their families and carers is at the heart of everything we do. We work closely with all other hospital professions and our community partners.

The post will form part of the Coastal Health and Wellbeing Leadership Team as funded from this establishment. This role will also provide an opportunity to be involved in trust-wide professional development projects and provision.

Main duties of the job

Supporting the development of the therapy provision at Dawlish Hospital. Working clinically to assess patients on the ward and implement person-centred treatment and individual rehabilitation goals. Facilitating people to leave hospital once they no longer have a criteria to reside. Reducing the use of Pathway 2 options and minimizing social care spend on discharge following therapy reviews. Understanding the barriers to timely discharge and finding ways to overcome them. Reducing hospital admissions and Length of Stay (LOS). Ensuring appropriateness of community referrals. Promotion and implementation of legal frameworks supporting discharge, ensuring multi-disciplinary teams understand the legalities of this framework to work confidently and promote a safe discharge. Leading a multi-disciplinary team responsible for the delivery and ongoing development to support patients' recovery in order to prevent a hospital admission or ensure a timely discharge back to their community and maximize their independent living to remain in their choice of home.

Job responsibilities

To undertake all aspects of practice as an autonomous practitioner, including professional and legal accountability for own work. To liaise with various multi-professionals, clients, and carers in situations which can involve highly complex, sensitive, and occasionally contentious information using well-developed communication skills. To be responsible for maintaining own competency to practice through CPD activities, maintaining a portfolio which reflects personal development. To be responsible for supporting specific abilities and interests of team members when identified by Team reviewers through performance review and personal development plans. To be responsible for the maintenance of professional status in line with HCPC recommendations. To work with other professional leads to support the wider multi-disciplinary team and to ensure that professional standards are met across the service. To actively participate and promote Safeguarding Adults, Mental Capacity, and Best Interests processes, including assessment and participating in meetings as required. To actively participate in resource allocation processes and attend meetings as required. To be responsible for/support the provision of rehabilitation for inpatients within the hospital settings as required for patient progression and discharge facilitation. Provision of Discharge to Assess visits in order to facilitate safe and timely discharges. Professional Practice Supervision will be available to the post holder.

Person Specification

Qualifications

Essential

Degree in Physiotherapy. HCPC registered. Desirable

Masters course level or equivalent level of experience. Knowledge and experience

Essential

Significant senior clinical experience of both acute and community therapy delivery. Extensive understanding of and implementation of the MCA 2005. Experience of staff resource management. Experience of working within the current hospital discharge policy. Desirable

Proven experience of project and change management. Previous experience as a team lead. Employer details
