Therapist – Primary Care Mental Health Professional

Open to nurses, AHPS, OT's, Therapists (CBT, Psychology/Psychotherapy), social workers etc. You would need professional registration though (NMC, VACP, BPS etc). Job Overview The successful candidate will join our Primary Care Mental Health (PCMH) service, which bridges the gap between NHS Talking Therapies and Community Mental Health Services. The service provides triage, assessment, signposting, referral, and brief NICE guidance interventions delivered through a person-centred and recovery-focused model. The Primary Care Mental Health Team is made up of skilled clinicians who are responsible for developing and improving services for people with common mental health problems in primary care. The role of the Primary Care Mental Health Professional involves working within the team to support adults over the age of 18 years, offering both one-to-one and group evidence-based psychological therapies. The role will include carrying a clinical case load and will be supported by the Team Manager and Clinical Lead. Main Duties of the Job This post holder will be working with a team that consists of a Team Manager, Clinical Lead, Mental Health Nurses and professionals, Access Practitioners, Clinical Care Navigator Manager, Mental Health Practitioners (ARRS), GP’s, Consultants, and Administrators. To be an experienced mental health professional with the ability to effectively screen, assess, and work with service users with: Complex and common mental health needs Stable, predictable, and enduring serious mental health needs Primary Care Mental Health Nurse Skills and Attributions: To deliver Primary care recovery-focused Psychological Interventions that meet the needs of service users who are too complex and/or not suitable for Talking Therapies yet do not meet the threshold for the Psychological Therapies in Secondary care mental health. Post holders will have a high level of autonomy working across designated GP practices, managing their own clinics. To review patients presenting with mental health issues, consider a range of options regarding treatment interventions, providing them with highly specialised advice concerning care when appropriate and liaising with GPs and MH teams. To work in partnership with service users to assess their mental health needs using a sound knowledge base and clinical judgement and where indicated deliver evidence-based brief interventions. To deliver Primary care recovery-focused Psychological Interventions that meet the needs of service users who are too complex and/or not suitable for IAPT yet do not meet the threshold for the Psychological Therapies in Secondary care mental health. To assist in the delivery of the ‘In-Reach’ Model of care that facilitates earlier discharge of service users from our Community Recovery Services, Early Access team, Home Treatment Team, Early Intervention, Older Peoples services, and Out-Patient Department. To deliver Wellness & Recovery Action Planning (WRAP) that supports a recovery-focused care model that emphasises the service user's choice, preference of interventions/treatment, and establishes meaningful networks of support that take into account their Bio/Psycho/Social needs. Support service users through their transition from secondary care mental health services and reduce the incidence of re-referral back into SMHS. Work autonomously and within Multi-Disciplinary and Place Based Team settings. Support Primary care (GP Surgeries) in the management of service users on Depot medication. Depots to be administered by practice nurses. Assist primary care with identifying early warning signs, relapse signatures, follow-up appointments following non-attendance, and re-referral back to SMHS where indicated. Support Primary Care with SEMI registers and enabling service users with Stable & Enduring Mental Health difficulties to access physical health checks working towards reducing the inequity in mortality and morbidity rates for this population. To signpost and support the seamless transition of care to other services including primary, secondary care, and voluntary services. Person Specification Essential Criteria NMC Registered Nurse in Mental Health or equivalent registration with the British Association for Counselling/ British Association for Counselling and Psychotherapy. Minimum 12 months post-registration experience in mental health. Minimum of 6 months community mental health experience is required. Degree in mental health nursing or equivalent e.g. psychological therapies.
