This role is a fantastic opportunity for apprentices who are currently studying for NVQ Level 2/3. Apprentices should note that the salary would be Agenda for Change Annex 21 trainee rate (75% max of band) until they have completed their apprenticeship. Interview Date: W/C 10th February 2025 Right Care is Barnsley's Single Point of Access for Health and Social Care staff supporting patient flow across Hospital and Community, coordinating admissions and discharges from Intermediate Care beds and Services, arranging support on discharge from hospital for Barnsley Residents. Due to a further expansion of Right Care Barnsley Patient Flow Coordination Team, we have a full-time post available and we are looking forward to welcoming a new person to join our friendly, successful team. We are looking for an individual who is willing to develop their knowledge and skills in relation to acute and community services. The successful person will need to work proactively as part of the wider team to deliver the Right Care Service benefit to patients and the public. For this role, it is essential that you are a motivated, enthusiastic, confident, and organized individual. It would suit someone with previous experience in the acute and/or community care setting. You should have excellent interpersonal, communication, decision-making, and influencing skills to ensure the highest standards of care are achieved. Main Duties of the Job

As a Patient Flow Coordinator, you will be based in the Right Care department and will deal with referrals taken electronically and over the telephone. You will be part of a wider team including Nurses, administrative officers, Social Care, and Intermediate Care staff. You will send referrals to a range of health and social care providers, including Care Homes, and you will follow up the referral to confirm service provision and inform the relevant teams. The aim of the service is to ensure that the right care is provided, at the right time, and in the right place. You will work closely with health and social care teams and professionals from primary, secondary, and community care, supporting the Right Care nursing and coordination staff to access care or other more suitable alternatives to hospital admission that will meet the needs of the service user. This role aims to provide comprehensive and proactive coordination and support, ensuring that all processes and tasks are undertaken efficiently, effectively, and to a high standard, contributing to the smooth running of the service and its overall effectiveness. Experience

Previous experience of working in a hospital setting. Previous experience of using Medway, Systm1, and other relevant EPR and clinical systems. Qualifications

At least 4 GCSEs (Grade A-C) or equivalent experience. NVQ Level 3 or willingness to work towards. Knowledge and Awareness

Able to organize own work and use own initiative. Sound understanding of confidentiality. Basic understanding of clinical governance. Understanding of patient flow. Understanding of community services and pathways. Understanding of Discharge to Assess. Trust Values
