JOB-20240905-7a6f785b Calling all Occupational Therapists ready for a new challenge in Community Mental Health! Join our dynamic team in Rochdale, located at Stansfield Place, Newbold, for up to 37.5 hours per week on a temporary contract running from January 2025 until the start of March 2025. Work hours are from 09:00 to 17:00, Monday to Friday, perfect for those who like their weekends free.

What you will do: Conduct assessments to determine the need for longer-term support under the Care Act 2014 and Care Programme Approach. Collaborate within a multi-disciplinary team to assess, plan, and implement brief, individualised care programmes. Assess occupational therapy needs and plan therapeutic interventions using the Model of Human Occupation, ensuring accurate record-keeping with up-to-date reports. Use standardised assessment tools like OSA, OCAIRS, and MOHOST to ensure outcome measures are evidenced, promoting partnership with relevant stakeholders. Develop partnerships with agencies, services, and individuals to support patients' optimal care. Engage in risk assessment and management formulation, fostering patient-focused interventions that respect social, cultural, and spiritual needs.
