Social network you want to login/join with: Jacksonville, FL 32257 Under the general direction of the Sr. VP Hospice Operations, this position will be responsible for the day-to-day home care team operations which include management of nursing practices, patient care delivery, the development of personnel, and compliance with regulatory standards, strategic planning and the fostering of interdisciplinary collaborative relationships within various divisions within the organization. Responsibilities:

Participates in recruiting, selecting, hiring, counseling and terminating staff within the division. Maintains quality staff by orientation, education, supervision and support. Evaluates staffing availability and reports to supervisor when adjustments are required. Monitors staff assignments based on census, acuity and geographic location. Monitors and maintains adherence to policies regarding attendance, time off requests and replacements. Monitors staff adherence to CMS, CoPs, and Florida Administrative Code regulations, and outcomes as measured by CAHPs. Demonstrates developmental action to encourage and prepare Patient Care. Administrator of additional management responsibilities. Establishes and manages goals and objectives through analysis and strategic planning. Anticipates customers’ needs and meets with center representatives to facilitate effective communication. Investigates and resolves customer concerns to ensure continuity of care. Actively seeks ways to develop professional relationships and promotes the organization program within centers and community. Collaborates with the divisional Medical Director and ARNPs to assure physician compliance with all federal, state and organizational standards, policies and procedures. Collaborates with other organizational personnel to develop effective models of quality care provision. Assists with budget preparation using organizational guidelines to ensure adequate resources for staffing, supplies and capital items needed for patient care. Assists with payroll process for Managers and Team Administrative Assistants. Completes performance appraisals for all direct reports in accordance with organizational protocol; oversees completion and reviews performance appraisals of all nursing and social services divisional staff. Communicates routinely with Managers and Clinical Resource Nurses for patient/caregiver status, care delivery concerns, treatment modalities and team trends. Ensures Clinical staff have the necessary resources to provide education that is current and relevant. Attends team meetings and leads Managers/Clinical Resource Nurse/IDG divisional meetings. Educates divisional staff in hospice, and associated topics relevant to the long term care and home environment. Reviews accident and incident reports for division, investigating issues/trends. Communicates with other organizational staff regarding patient status/concerns in accordance with organizational protocol and HIPAA regulations. Participates in various organizational committees and task forces as well as community based committees which benefit patient care and relationships. Provides emotional support to divisional staff, patients and caregivers. Creates and maintains a professional, comfortable atmosphere reflecting standards of quality patient care. Participates in public speaking, community relations events and educational seminars. Leads divisional staff by exhibiting teamwork, motivation, change management and effective, timely communication. All other duties as assigned. Qualifications:

Required Licenses/Certifications:

Current licensure as registered nurse in state of practice. Current State of Florida driver’s license with driving record acceptable to Community Hospice insurance carrier. Education/Experience:

Any combination of education and/or experience that would provide the required skill and knowledge for successful performance would be acceptable. Typical qualifications would be equivalent to: Possession of a degree from a diploma nursing program, two (2) year Associate’s Degree, or Bachelor’s Degree in Nursing, Master’s Degree preferred. Minimum of four (4) years nursing experience. Previous Hospice Care administrator experience. Minimum of two (2) years of nursing administration. Knowledge/Skills/Ablities:

Principles and practices of nursing. Hospice philosophy and standards. Skilled at:

Communicating and listening effectively. Supervision of staff. Usage of computer.
