Community Care Nurse Specialist

(CCNS) to work as part of the CCNS Team. This is an exciting position. St Barnabas Lincolnshire Hospice has been working for the past 4 years with Adult Social Care and developed a truly innovative and exciting specialist nurse role to work at Pilgrim Hospital Boston, in collaboration with a fellow Community Care Nurse Specialist based at Lincoln. The role enables people in their last year of life to have their palliative and end of life care needs identified much earlier. This in turn improves person-centred outcomes, including symptom management and wellbeing for people by enabling timely access to palliative care in the community including hospice care. Community Care Specialist Nurses work as part of the Adult Social Care Team within the wider multidisciplinary team, and when necessary, work alongside other clinical staff to support senior clinical decision-making. The Community Care Specialist Nurse uses advanced knowledge and skills to facilitate complex communication and care planning in the community for people who often have their complex needs associated with palliative and end of life care needs missed. The service has been commissioned by Adult Social Care through St Barnabas Lincolnshire Hospice. The partnership continues to work together with key stakeholders to develop the service and support its implementation across Boston and Lincoln ULHT sites. Main duties of the job The primary focus of the CCNS role is to support the timely identification of people in their last year of life and facilitate and broker person-centred palliative and end of life care within the community and operational areas of St Barnabas Hospice to improve both person-centred and system outcomes. The post holder will: Deliver evidence-based palliative and end of life care and support improvements in the experience for families. Provide support/education to colleagues and other health and social care professionals within the multidisciplinary team. Support the delivery of key quality and performance indicators relating to this service. Practice at a specialist level, including person-centered care, therapeutic skills, and triadic relationships, sharing knowledge, best practice and critical reflective practice.
