Livewell Southwest is an independent social enterprise (Community Interest Company) providing integrated health and social care services for people in Plymouth, and community health services in South Hams and West Devon as well as some specialist services for those living in other areas of Devon and Cornwall. The post holder is responsible for the delivery of high-quality evidence-based care for acutely ill patients within their homes, with the aim of preventing hospital admission or facilitating rapid discharge. The post holder will liaise with the referring clinician when accepting patients for treatment and be supported by senior clinicians and the clinical manager. Responsibilities include effectively assessing, planning, implementing, monitoring, and evaluating patient care, which includes admitting and discharging patients from the caseload. To prevent and reduce hospital admissions and minimize hospital lengths of stay, the post holder will provide intensive and advanced nursing input and involve appropriate members of the multi-disciplinary team. The post holder is also responsible for coordinating a shift in the absence of a senior clinician. The Urgent Care Nursing team works on a flexible rota shift system, covering 24 hours per day over seven days a week. The post holder will undertake and develop all nursing skills and competencies as required for their role to enhance the skill mix of the nursing team, ensuring that the ever-changing health needs of patients are met. They will provide the interface between hospital, primary care, community, and social care settings.
