This role involves providing essential mental health support to older adults experiencing a mental health crisis within the community. Key Responsibilities:
Visiting older adults in the community who are experiencing a mental health crisis. Managing cases effectively in a community setting, reducing the need for hospital admissions. Delivering high-quality, patient-centred care as part of a multidisciplinary team. Maintaining detailed documentation and working within NHS guidelines. What We Offer:
