Job summary We are a well-established practice working across 2 sites with a list size of over 19,000 patients and we are looking to recruit an enthusiastic and dedicated Diabetic Specialist Nurse to complement our friendly and dynamic team. Main duties of the job The successful candidate would work to ensure a high level of care is delivered to our diabetic patients, including initiating insulin. The practice is very supportive of any development opportunities, but is looking for a clinician that can really hit the ground running! This permanent post can offer a wonderful working atmosphere in a friendly and supportive team! The position is part-time (hours/week). About us We are a training practice and our forward-thinking multi-disciplinary team includes GPs, Pharmacists, Paramedics, Practice Nurses, HCAs and Phlebotomists, all supported by a strong administration. Job responsibilities To help lead the practice's diabetic service delivery. This includes managing patient diabetes holistically and supportively, and working with patients to manage their treatment plans. Occasional home visits will be required. To work alongside the Community DSN in joint clinics, as per the Local Diabetic LES. Experience Desirable Experience of working in a primary care environment, as a practice nurse or community nurse Experience of diabetes and other chronic disease management Other Essential Hold a current driving licence with business insurance Sole and exclusive use of a suitable Motor Vehicle Enhanced DBS disclosure. Desirable Vaccine and immunity status in accordance with UK Department of Health Guidelines Able to deal with sensitive issues such as sudden death/bereavement, child abuse, domestic violence Able to adjust and respond in rapidly changing environments Able to work in a calm and organized manner in situations of extreme workload Personal Qualities Essential Able to work cooperatively with the multi-disciplinary team Able and willing to develop professionally Able to use own initiative Qualifications Essential Current registered Nurse (NMC) Desirable Nurse Independent Prescriber Formulary Teaching and assessing qualification
