What you will do:
Deliver expert clinical pharmacy services to patients across various departments
Collaborate with healthcare professionals to optimise medication management
Provide specialist advice on pharmaceutical care plans
Monitor patient outcomes and adjust treatment plans as necessary
Ensure compliance with healthcare regulations and standards
Participate in teaching and mentoring junior staff
Contribute to the development of clinical guidelines and protocols
Requirements:
We are looking for an experienced pharmacist with at least 2 years of hospital experience, including clinical pharmacy practice. Previous work within specialist areas of pharmacy practice is highly desirable. Although not mandatory, having a valid UK driver's license is preferable for working across multiple locations. You must be HCPC registered to apply.
