We are looking for an enthusiastic and dependable Care Navigator to join our team. The position is available to work 16.75 hours per week between our Fulwood and Ingol sites, Monday to Friday (Friday Day Off). You will be required to cover holidays and sickness at short notice and be as flexible as possible. Applicants need not apply if they are unable to cover holidays and sickness. You will be passionate about maintaining high quality and effective customer/patient services. Experience working within General Practice is advantageous but not a prerequisite, as full training will be given and you will be working alongside supportive colleagues. Working as part of our Care Navigation Team, you will be assisting patients on reception with appointments, prescriptions, and queries, plus also supporting our clinical team with all the administrative tasks that come with running a busy GP practice. Hours

Monday 13:00-18:00 Tuesday 08:00-12:00 Wednesday 08:45-12:30 Thursday 14:30-18:30 Friday DAY OFF You will also be on a rota with 15 other members of staff to work Saturdays, 9am-12pm (rate: time and a half). About Us

Working over our 2 sites (Ingol & Fulwood), you will be a member of a very dedicated team which includes 8 GPs, 1 Advanced Nurse Practitioner, 4 Practice Nurses, 1 Nurse Associate, 1 HCA, 1 Clinical Pharmacist, and a team of Pharmacy Technicians, plus a very supportive and friendly administrative team. Job Responsibilities

Develop and maintain effective working relationships with all other employees and contractors at the Practice. Support and uphold an open, positive and honest working culture. Answer the telephone to enquiries and make outgoing calls to patients and other agencies as required. Greet patients in person, working to put them at ease and giving any explanations within your competency. Make and change appointments as required. Use email and phone to communicate with patients and other agencies. Keep the reception and waiting areas in a smart and orderly fashion. Handle payments to the practice by cash and cheque. Keep records of financial transactions. Complete insurance forms and other types of patient plan forms. Report all significant events, including near misses for inclusion in the practice Risk Register. Practice the highest standards of confidentiality when dealing with all aspects of patient information. Participate in activities required to assess compliance with CQC Fundamental Standards. Participate in the maintenance of a Practice Complaints System, and in resolving complaints, and in improving service. Use all equipment in accordance with published instructions and guidelines. Report to the Practice Manager any faults or safety worries with equipment and the premises. Open and distribute practice post. Liaise with secondary care providers and arrange transport as necessary. Deal with repeat prescription requests. Assist patients with registrations and processing the paperwork. Deal with all incoming and outgoing patient notes. Maintain patient electronic medical records, accessing information when required. Daily preparation of Doctors' rooms and other clinics, ensuring surgeries are stocked with the necessary paperwork and forms to enable their surgeries to proceed without interruption. Maintain and encourage adequate security measures, ensuring all clinic rooms are locked when not in use. Support and encourage other members of the Practice team. Maintain the Health and Safety of all staff, patients, and visitors. Any other duties as requested by the Doctors or management team. Person Specification

Qualifications

Essential Educated to GCSE level standard. Desirable Relevant administration qualification. Experience

Essential Minimum of 12 months working directly with members of the public in a busy environment. Experience of setting up general office administrative systems/working in an administrative office environment and working as part of a team. Desirable Experience of working in a GP practice. Experience of the EMIS web clinical system. Skills & Abilities

Essential IT skills. Intermediate keyboard skills. Literacy skills (spelling, comprehension, etc.). Numeracy skills. Organisational/problem-solving skills. Verbal communication/reasoning skills. Ability to deal with people in person and on the telephone. Desirable Experience of Microsoft Office applications.
