Job summary An exciting opportunity has arisen for a highly motivated Registered Nurse or Social Worker with a desire to optimize patient flow and discharge pathways across the region, working closely with multi-disciplinary colleagues. The successful candidate must display a considerable degree of initiative, possess mature judgement, and provide comprehensive, professional, operational, effective leadership and managerial support within the Single Point of Access and Patient Flow Service, while being supported by the Service Manager. You will also work closely with the Service Manager and deputize for them at times, ensuring a seamless service is delivered to patients across specialties, ensuring optimum bed capacity and that resources are deployed to meet the challenges faced with patient flow. The successful candidate will also have the opportunity to work alongside six different NHS Trusts across East of England. Main duties of the job The post holder will be responsible for reviewing and processing day-to-day coordination of requests for CAMHS, Adult Eating Disorders, and Secure inpatient admissions. The Patient Flow Bed Management Team will manage bed resources across the East of England (EoE) provider collaborative. Provide clinical advice to all referrers from across the EoE collaborative regarding admission processes. Receive referrals from EoE clinicians requesting hospital admissions, negotiate appropriate placement considering service user presentation, organizational policy, and resources, and take clinical responsibility for bed allocation decisions. Liaise with services across the collaborative to facilitate the organization of EoE resources to ensure the availability of beds for admissions, including supporting discussions and decisions regarding transfers between wards and to community services. Work proactively with EoE services, social care services, and other providers to support discharge, transition, and contingency plans to ensure bed availability across services in scope. Jointly with the bed management service and outreach teams, develop repatriation plans for YP currently placed out of the EoE area, where clinically indicated. Demonstrate clinical excellence in contributing to the management of complex cases and assist inpatient MDTs in the management and treatment of delayed discharges and Out of Area Treatment processes (OATs). Support the bed management/outreach team manager to ensure robust and timely clinical audit systems and the production of associated reports. Provide clinical supervision to junior staff within the team. The post holder will regularly undertake QI and development activity as part of the role. Person Specification

Qualifications Essential Professional qualifications RN with mental health experience or DipsSW or CQSW or OT. Relevant degree or equivalent post-qualification training. Management Qualification or equivalent management experience to master's level. Evidence of professional registration. Desirable Approved Mental Health Professional. 3 years senior level experience at band 7. Skills and Knowledge Essential Strong Leadership and Motivational Skills. Demonstrate a high level of clinical expertise. Demonstrate sound organizational skills and the ability to lead teams. Knowledge of legislation and practice relevant to the care group. Ability to assess needs of service users and their carers and to implement care planning within the Care Programme Approach framework. Ability to work collaboratively with other agencies and as a member of the multidisciplinary team to influence practice in relation to early discharge and bed occupancy. Ability to use the Trust computerised record system, Care Notes. Understanding of mental health community-based services. Analytical skills Essential Ability to consider more than one solution to a potential problem. Innovative thinker with the ability to cut through barriers to change. Positive and outcome-focused approach. Ability to take decisions when on call at night and weekends. Communications Essential Excellent communications skills, oral and written. Budgetary management Essential Knowledge and understanding of Health and Social Care budgets and pressures. Manage a budget up to £. Experience Essential Experience of working with people with mental health problems and in a care environment. Experience of working in a mental health setting as a ward manager or equivalent experience in the community relating to the discharge from hospital of service users. Knowledge of the National Service Framework and understanding of clinical governance. Experience in extra contractual referrals (ECRs) and Individual Patient Agreements (IPAs). Desirable Previous experience training/practising as an ASW/AMHP. Experience of using a variety of intervention methods. Car driver (unless you have a disability as defined by the DDA).
