e7041df98015 Job Description: We are looking for an experienced and passionate Health and Wellbeing Coach. About the Role Are you passionate about supporting individuals to overcome challenges and improve their wellbeing? As a Health and Wellbeing Coach, you will be part of a multi-agency team delivering high-quality, person-centred support to individuals serving sentences in HMP Durham. Working in alignment with the Dynamic Probation Framework model, your role will focus on addressing personal wellbeing issues and supporting individuals to make positive changes in their lives. You will manage a caseload of service users, delivering group sessions and 1:1 support tailored to individual needs, and collaborate with other professionals to achieve the best possible outcomes. Key Responsibilities

Provide person-centred support to individuals transitioning from prison settings or serving community sentences. Facilitate group sessions and 1:1 support in a responsive, flexible, and adaptable manner. Address personal wellbeing issues and guide service users toward positive changes in their lives. Work collaboratively with professionals from various teams to deliver seamless support and ensure optimal outcomes. Build and maintain strong working relationships with service users and stakeholders, providing guidance and encouragement throughout their journey.
