The CMRN is a unique collaborative integrated pathway delivered across multiple organisations, hyper-acute, spoke, extended and community services, providing specialist rehabilitation to patients who have suffered traumatic injury or illness dependent on need and not diagnosis. The post will be primarily based at The Walton Centre. There may be a requirement to work across and support our other network units based on Broadgreen, St Helens or Wirral sites. As a Rehabilitation Coordinator, you will play a pivotal role within the multi-disciplinary team, acting as a key worker to a designated caseload and advocate for patients, families and carers to navigate them through their rehabilitation pathway. You will have excellent communication skills as liaison with referrers, and working closely with the multidisciplinary team including Consultants, Nurses and Allied Health Professionals is an integral part of the role. Qualifications
Nursing or AHP degree or equivalent relevant professional qualification NMC/HPC Registration Post Graduate training within Rehabilitation at specialist level Knowledge & Experience
Demonstrable post graduate experience in Rehabilitation at specialist level Previous extensive experience of treating patients with neurological and or multiple conditions Ability to prioritise workload Ability to demonstrate effective team working within a multi-agency/network framework and working in partnership with a multi-disciplinary team Clinically relevant experience in neurology or trauma specialties within a rehabilitation setting Managing and interpreting data Evidence based and experience of reflective practice Understanding the Mental Health Act 2007 and Mental Capacity Act Understanding of clinical audit and research practices relevant to rehabilitation Participation in change management Skills & Attributes
