Job summary The post holder is an experienced Pharmacist, who acts within their professional boundaries. They will work as part of a multi-disciplinary team in a patient-facing role. In this role they will be supported by a Senior Clinical Pharmacist and GP who will develop, manage, and mentor them. The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will respond to daily queries from patients, action medicines reconciliation on transfer of care and clinic letters. They will undertake medication reviews across different disease areas. Clinical Pharmacist- Prescriber:
The post holder will be an independent prescriber and as such will be expected to work within their scope of practice and develop this scope of practice to meet the requirements in primary care. Clinical Pharmacist- Non-Prescriber:
The post holder will be supported to develop their role to become an independent prescriber. Main duties of the job Patient facing clinical medical reviews Management of medicine at discharge from hospital Medicines advice to practice staff and patients Ensuring clinical documents received within practice are actioned appropriately Contributes pharmaceutical advice for the development and implementation of new services that have medicinal components Works with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved. Job responsibilities Clinical Pharmacist- Prescriber/Non-Prescriber Salary: £44,345-£53,045 per annum, FTE Hours: Full time hours per week or Minimum 30hrs per week Location: Practices of Living Well PCN Southampton Accountable to: Senior Clinical Pharmacist Key Responsibilities: Review the ongoing need for each medicine, review monitoring needs and support patients with their medicines ensuring they get the best use of their medicines (medicines optimisation). Seeking support from senior pharmacist/GP as required. Supports medicines reconciliation following patient discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes. Works with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Answers all medicine-related enquiries from GPs, other practice staff, other healthcare teams (community pharmacy) and patients. Suggests and recommends solutions. Provides follow up for patients to monitor the effect of any changes that are made. Responds to daily eConsults, identifying any red flags and underlying serious pathology and takes appropriate action including referring to other members of the primary care team as needed. Works as part of the pharmacy services team ensuring that any clinical documents received within the practice are actioned appropriately, particularly those associated with medication changes and monitoring within own scope of practice. Person Specification Essential Qualifications: Clinical Pharmacist- Prescriber: Masters degree in pharmacy (MPharm) (or equivalent) Mandatory registration with the General Pharmaceutical Council (GPhC) Independent prescribing qualification and annotation on GPhC register Specialist knowledge acquired through post-graduate diploma level or equivalent training/experience Clinical Pharmacist- Non-Prescriber: Masters degree in pharmacy (MPharm) (or equivalent) Mandatory registration with the General Pharmaceutical Council (GPhC) Essential Aptitudes: Ability to organise and prioritise workload effectively Ability to exercise sound judgement when faced with conflicting pressures Excellent written and verbal communication skills IT literate (EMIS and S1 training will be provided) Experience and enthusiasm for clinical teaching of multi-disciplinary students and qualified staff when appropriate Will undertake the CPPE Pathway for Pharmacists as part of this role Essential Attitude/Commitment: Good communication and empathy skills with patients Evidence of willingness to participate in a multi-disciplinary team environment to provide an integrated service for patients Ability to work as a member of a team, both through communication and sharing workload Willingness to work flexibly to deliver clinically effective and cost-effective healthcare Essential Experience: Clinical Pharmacist- Prescriber: Specialist knowledge acquired through post-graduate diploma level or equivalent training/experience In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare Minimum of 3 years post qualification experience Clinical Pharmacist- Non-Prescriber: Specialist knowledge acquired through post-graduate diploma level or equivalent training/experience In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare Minimum of 2 years post qualification experience Please note that if you are NOT a passport holder of the country for the vacancy you might need a work permit.
