Job summary An exciting opportunity has arisen for an appropriately skilled and experienced registered nurse to join Bradford Health and Care Partnership's Continuing Healthcare (CHC Team) as a Clinical Lead/Complex Case Manager. The post holder will lead the development and management of a coordinated approach to NHS Continuing Healthcare (CHC) and Funded Nursing Care (FNC) within a nursing home, registered or community settings including supported living, including the assessment and review of people potentially eligible for, or already in receipt of, NHS Continuing Healthcare or Funded Nursing Care. Main duties of the job The post holder is expected to supervise, and line manage clinical and non-clinical staff and undertake responsibility for managing and coordinating assessments, ensuring the development and ongoing reviews of specialised packages of care where the Health and Care Partnership is the lead commissioner or funding agency, and assist with the application of National Frameworks and local policies in relation to NHS Continuing Healthcare and Funded Nursing Care. The post holder will be required to provide expert advice and identify appropriate providers to address the needs of complex individuals. Autonomously manage a case load of individuals with highly complex health needs providing significant clinical guidance to providers and other stakeholders. Ensure the Mental Capacity Act is considered and ensure relevant processes are followed. Qualifications Essential Educated to post registration/degree level or equivalent knowledge and experience. Post Graduate qualification or specialist training in District Nursing. Current and relevant registration in nursing. Desirable Recognised teaching/mentoring qualification or equivalent experience. Independently mobile to work across several sites across West Yorkshire and travel as required. Experience Essential Specialist/Expert knowledge of complex care needs and complex case management. Further knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent. Experience of multidisciplinary team working, knowledge and understanding of health and social care processes for discharge planning. Detailed knowledge of the National Framework for Continuing Health Care October 2022 (Revised). Knowledge and understanding of the Mental Capacity Act 2005 and Mental Health Act 1983 and its application. Ensure that the principles of the DOLs and the MCA are embedded in local policies and procedures, are applied within the CHC Team so that they become business as usual when designing and developing. Knowledge and understanding of Personal Health Budgets. Evidence of ongoing professional development. Experience of leading and managing staff. Experience of communicating and engaging effectively with external agencies/stakeholders including local authorities. Understanding and knowledge of the workings of the NHS, the challenges it faces and of the reform agenda. Experience of using performance management/improvement systems. Desirable Some prior experience of working within function. Experience of working in the NHS. Some knowledge and experience of programme and/or project management.
