What you will do:

Assess and evaluate patients' physical conditions and create personalised treatment plans to enhance recovery and promote independence. Collaborate with a multidisciplinary team to deliver comprehensive discharge planning and ensure smooth transitions for patients returning home or to community care settings. Provide expert advice and education to patients and their families, ensuring they have the knowledge to continue rehabilitation safely and effectively. Document patient progress accurately and maintain detailed records to support ongoing care and future assessments. Participate in meetings and case discussions to share insights and develop best practices within the team.
