To provide comprehensive, efficient and effective administrative service within a Transfer of Care Hub to support the discharge system and data outputs as required by its multi-Disciplinary teams both internal and external. Using your organisational, analytical, administration and people skills, you will contribute towards an effective and consistent level of service. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes. Main duties of the job
Update, collate and share complex discharge data within the service unsupervised. Organise and prioritize daily tasks unsupervised. Input and maintain information on the service’s various systems, ensuring accuracy and timeliness. Create and maintain data logs using MS Excel and linked NHS systems. Monitor patients added to or removed from discharge logs. Manage data entry, retrieval, and maintenance of patient records. Handle a high volume of queries via secure email, liaising with both internal and external agencies. Provide effective liaison with multi-department staff, patients, carers, and outside agencies. Answer the telephone and respond to queries in a timely manner. Collect and input data on to the Emis Web system, investigating discrepancies. Participate in service development activities and mandatory training. Assist with the training of new clerks within the service. Liaise with colleagues at all levels across other agencies and organisations. Manage workload and prioritise tasks unsupervised. Operate and maintain information systems, ensuring high standards of information recording. Produce standardised reports and/or statistical information as requested. Transfer relevant information to all health and social care providers as requested. Input accurate and timely information required for audit purposes. Provide cover for and undertake duties of absent colleagues. Support individuals’ equality, diversity, and rights. Contribute towards service improvement initiatives. Provide information and support to service users/carers as appropriate. Contribute to the continued improvement and quality of the administrative support service.
