What you will do: Lead and develop clinical pharmacy services tailored to frailty, elderly, and stroke patients, enhancing patient care pathways. Collaborate with multidisciplinary teams, providing expert clinical pharmacy services in emergency care and frailty settings. Mentor junior pharmacists and support the strategic advancement of medicines management. Deliver specialized pharmacy services to the Women and Children's division, optimizing medication usage through guideline development. Lead the respiratory medicine pharmacy service, engaging in team meetings and contributing to patient management plans. Offer expert clinical pharmacy services in the cardiology department, focusing on safe and effective cardiovascular medication use. Requirements: HCPC registration required.
