The Learning Disability Social Work Team is part of North Lincolnshire Council’s Adult’s and Community Wellbeing support offer. Staff work across the community providing services in an individual’s home, care home, community setting or place of choice. The team provides proportionate support at the right time and place to ensure maximum independence. The Learning Disability Social Work Team offers a service focusing on North Lincolnshire Council’s statutory responsibilities under the Care Act 2014 and the Mental Capacity Act 2005. Who we support

Adults residing in North Lincolnshire Those eligible under the Care Act and who have a primary diagnosis of a Learning Disability A day in the life…

Our primary focus is supporting people with a Learning Disability. We adopt a multi-disciplinary approach to ensure that the health and social care needs of people living in North Lincolnshire are met in a co-ordinated way. We work closely with external partners such as police, fire, hospitals, community learning disability teams and private/third sector agencies to support people to feel safe, supported and in control of their own lives. The Learning Disability Social Work Team works with people and their circle of support to develop support to improve and meet social care needs, help with daily living activities, practical tasks and develop the confidence, strength, and skills to carry out these activities independently to enable people to continue to live their lives well. Our goal is to provide a service that is fully person-centred, supporting people’s physical, emotional and social needs to improve and develop their whole wellbeing. Improve health and well-being outcomes Increase and sustain daily living skills Support carers to continue to care Support and encourage involvement and the use of community assets Enable independence, ensuring individuals are supported actively to take managed risks to build confidence and increase independence. Assess need under the Care Act eligibility criteria and complete Individual support plans, in partnership with individuals and their circle of support to ensure the support we provide is personalised and effective at an individual level to achieve good outcomes and maximise independence. Undertake assessments to identify needs Co-produce individual support plans Undertake reviews Conduct Mental Capacity Assessment and make best interest decisions Offer advice and guidance Commission packages of care to meet identified needs Participate in and contribute to Section 117 reviews (Mental Health Act 1983) What qualities does your team look for in someone wanting to pursue a career in this area?
