What you will do: Undertake scheduled reviews and monitoring visits to residential and nursing homes. Promote and enhance the lives of people living with dementia within residential or nursing placements. Work closely with providers to improve the opportunities available to people living with dementia within placement settings. Provide detailed reports to the Team Manager regarding visits and interactions with placement settings.
