CCS Luton Community Nursing has transformed its Community Nursing Services through the development of cluster working, dividing planned and unplanned care, triaging of referrals and through the introduction of a Single Point of Access (SPA). We are now entering a new phase of transformation and innovative partnerships with local Primary, Secondary and Social Care organisations. We provide a comprehensive 24 hour a day service.
This is an ideal opportunity for Staff Nurses to develop their skills within the community setting providing holistic care to housebound patients.
Experience of working in Luton would be an advantage although full induction and training will be given in line with service need.
Luton has good transport links to the M1 and M25 and the London Luton Airport. In conjunction with Luton Borough Council, parking permits are available for staff to use in and around Luton.
Main duties of the job
To support the provision of Community Nursing care within the Integrated Community Nursing Team, providing scheduled and unscheduled care to the house bound population of Luton.
To assist in the assessment, planning and implementation of holistic care provision.
To work with the MDT to prevent avoidable hospital admissions.
You will need to be adaptable to overcoming challenges and ensuring that the patients receive the evidence based nursing care they require and further refer onto other services that are identified to be appropriate.
Supporting other colleagues and embracing service development is fundamental to the role.

Job responsibilities
To ensure at all times patients are treated with care and compassion.
To support the Senior Nurse in organisation and coordination of the overall care to meet clients care needs, which may be complex involving other professionals and agencies in an integrated and co-ordinated way.
To accept responsibility for those patients whose care has been delegated to them by the Community Nursing Sister or Senior Nurse and ensure continuity of the nursing care by on-going assessment.
Escalate risk as appropriate.
To deputise for the Community Nursing Sister in their absence as required.
Assess and prioritise unscheduled requests for nursing intervention and allocate to the appropriately skilled clinician as appropriate.
To refer patients to other services / agencies as appropriate.
To organise own work as delegated by the Senior Nurse.
Delegate work to junior staff and learners as required.
To assist in the planning and co-ordination of duty, covering annual leave, sickness and study leave under supervision of a Senior Clinician.
To assist in the assessment and implementation of care interventions to meet identified health needs of individuals, families and communities.
Maintain associated records.
To provide mentorship to pre- and post-registration nursing students.
