The post holder is responsible for the delivery of nursing practice within socially vulnerable communities and ensuring that everyone is treated with dignity and needs are met in a timely manner. The focus of the role is to bridge the gap of health inequalities and provide education to the communities and professionals of the social determinants within vulnerable groups. To develop and maintain close relationships with Primary and Secondary Mental health teams and staff from a range of statutory and nonstatutory services, providing advice and support to promote the management of those with mental health needs. As an autonomous practitioner, the post holder is responsible for the care delivered, demonstrating critical thinking, sensitivity, and skills in clinical decision-making in the management of patients. You'll ideally have previous experience in a community setting with a knowledge and understanding of culturally sensitive trauma-informed approaches. The practitioner will work collaboratively and maintain liaison and co-operation with other statutory, voluntary, and contracting agencies to ensure optimum level of care provided with other partners whose service delivery has a direct impact on Asylum Seekers and Vulnerable groups.
Main duties of the job Support the Senior Nurse in delivering and supporting effective care to patients. Assist with managing the healthcare service across the socially vulnerable groups that we support. To deputise for nursing colleagues during any periods of absence. To represent the primary and community care viewpoint at operational and strategic meetings when necessary. Work closely with the wider multidisciplinary team to ensure safe effective delivery of health care, including mental health and public health services. To act as an accountable role model and clinical expert for healthcare. Responsibility for the promotion and use of the audit cycle within clinical practice. Support the development and implementation of Healthcare Policies and procedures. Responsibility of offering the initial assessment/completion of the All Wales Blue book to Asylum seekers living in ABUHB Locality. Responsibility of providing cultural awareness training and needs of the socially vulnerable groups to other disciplines for example g.p practices staff, mental health teams, and Accident and Emergency staff. Work in conjunction with other disciplines in the assessment, planning, implementation, and evaluation of healthcare service provision to ensure a smooth patient pathway through the services.
