specialist multi-disciplinary team (MDT) , providing efficient, patient-focused care to promote recovery and support discharge planning. You will play a pivotal role in delivering high-quality therapeutic management to patients within our frailty pathway. Main duties of the job

Conduct advanced holistic triage and assessment for patients with complex physical, emotional, and psychological symptoms. Develop and deliver individualised treatment programmes. Act as an advocate for patients and carers, promoting their needs within the MDT. Use evidence-based practice to ensure high standards of care. Provide supervision and support to junior staff, students, and MDT colleagues. Participate in risk assessment and management, adhering to Trust and National protocols. About us

Experience

Previous Band 5 Physiotherapy experience, with exposure to complex symptom management and long-term health conditions. Qualifications

Current relevant professional registration (HCPC Registration). Degree or equivalent in Physiotherapy. Disclosure and Barring Service Check
