Practice Nurse

·
Part time
Location: Long Stratton
·
Job offered by: NHS
·
Long Stratton Medical Partnership has an exciting opportunity for a practice nurse who has experience in primary healthcare to join our excellent practice, on a part-time basis for 24 hours per week. This will be working over three days and shifts can be discussed at interview. The ideal applicant will need to be flexible and be able to cover for absences and holidays. Working under the direct supervision of the lead nurse, you will assist the clinical team in the provision and delivery of prescribed programmes of patient care. Main duties of the job

Responsibility for meeting the nursing care needs of patients identified by the nurse, or where care has been transferred to the nurse by the GP. Triage of patients seen in the Treatment Room, or by telephone; offering advice, nursing treatment and/or emergency or routine referral to the doctors, specialist nurse, or other agencies if appropriate. Responsibility for identifying care needs within the practice population, by practice profile or audit; and organising work to best meet the needs identified. Managing nursing care of those patients with chronic disease, operating specific clinic sessions if needed, and maintaining an appointment and recall system for patients. About us

Long Stratton is a small town in South Norfolk, 10 miles from the historic city of Norwich. We have 11,500 patients on our list, a CQC rating of Good and operate from both our main site and a branch site in Newton Flotman. We are also a dispensing practice with a dispensing list of around 5,500 patients. We use EMIS Web and run cloud-based telephony which links seamlessly with the clinical system and provides excellent functionality. We have completed our surgery expansion work with a new wing being added to include 7 clinical rooms. The reason for the expansion is that Long Stratton is being bypassed, 2,000 new homes are planned and therefore potentially 5,000-6,000 new patients will be joining us. We offer a comprehensive benefits package including 5 weeks annual leave (plus bank holidays), the NHS pension scheme, and an Employer Assistance Programme for the employee and family members. We also offer free on-site car parking, team building and social activities and access to NHS discounts. We are innovative, forward-thinking and embrace change. Job responsibilities
