NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024, but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week. Car Driving is Essential for this Post The main purpose of the Practitioner service is to provide a high quality, easily accessible, timely and effective clinical service for patients within Midlothian. You will work effectively as a member of the team, ensuring optimal patient care in the patient’s home environment. The post holder will utilise clinical skills and knowledge incorporating a high level of critical thinking and clinical judgement skills. They will lead on developing an impact evaluation on this service, working autonomously with a high level of decision-making and problem-solving. They will lead this service improving practice underpinned with values-based care. Responsibilities include providing clinical support and advice to other team members while acting as a first point of contact and making referrals to or coordinating with other members of the multi-disciplinary team. Possess the full range of skills and competences to meet and prioritise emergency and urgent care needs of patients who are within the care of the Hospital at Home team, covering a wide range of interventions and having the capacity to call in specialist expertise when required. Ensure the highest standard of personalised care is delivered to patients and families, working in partnership with the multidisciplinary Midlothian teams. Assess, plan, implement advanced level care, and deliver and maintain optimal care outcomes. Motivate staff to provide high standards of care by acting as a role model. Provide advanced and specialist education and training programmes for the team. This post requires the post holder to have a PVG Scheme membership/record. If the post holder is not a current PVG member for the required regulatory group (i.e., child and/or adult), then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
