As part of the One Door Chronic Disease Management Team, the Chronic Disease Management (CDM) Dietician uses clinical practice guidelines to provide nutrition care and education to patients/clients with a variety of clinical conditions that require nutritional intervention. The CDM Dietician will deliver services to a variety of patient populations including high risk groups suffering from chronic diseases and/or complex multi‐system diseases. This role will also provide health education to patients with a focus on health promotion, disease prevention, and chronic disease management. The CDM Dietician is a part of the Primary Health Care Program and reports to the Primary Health Care Manager. This position provides services in Primary Care Centers throughout the Pictou County area; travel will be required. About You

We would love to hear from you if you have the following: Bachelor of Science in Human Nutrition Completion of Dietetic Internship accredited by Dietitians of Canada Licensure by Nova Scotia Dietetic Association required 3 years recent related experience required working in a Primary Health Care setting/community based team/community setting and experience with people living with a chronic condition Experience with individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles Designation as a Certified Diabetes Educator preferred or willingness to obtain within first year Willing to complete requirements for Delegated Medical Function Preferred experience in partnership development; stakeholder collaboration, and community development Understanding of population health, health promotion strategies, and behaviour change principles In-depth knowledge of local health network and community based services Demonstrates person-centred / family-focused, diversity relevant approaches to care Well-developed interpersonal, teamwork, oral and written communication skills Demonstrates flexibility, adaptability and willingness to change Demonstrates innovation in program planning, implementation and evaluation, and quality improvement planning skills Demonstrated proficiency in Microsoft Office computer applications; familiarity with the Meditech patient information system and myNightingale electronic patient record strongly preferred. Annual CPR-AED certification and annual WHMIS training and certification Valid driver's license and access to a reliable car for travel for work Competencies in other languages preferred; French an asset Please ensure your resume is up to date and includes all relevant education, experience, training, and certification. Permanent Part-time position; 45 hours bi-weekly. Shifts may include evenings, weekends, and holidays. Compensation and Benefits

$34.78 - $45.85 Hourly. Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
